The initial evaluation rate is $432 (first 2 hours, same day). This rate includes includes a formal written report with recommendations and a 45-minute consultation. (Should more assessment time be needed, often across additional days, this is processed separately at a rate of $150 per hour.) 

 

To streamline communication and transactions around therapy billing, Albers Therapy Group would like to store your credit card information within our secured server. All information is maintained securely through our electronic medical records (EMR), scheduling, and billing system and/or through our merchant processing system with Wix and JotForm. These elecronic forms are HIPPA compliant, encoded, and protected.  All accounts are required to have a credit/debit card on file. If you require special payment arrangements (other than AutoPay via credit/debit card) you must receive prior approval from the office. A special payment agreement must be signed in order to accept any other terms outside of our credit card policy. We reserve the right to adjust service rates. There will be advanced notice of this prior to billing. By providing your credit card information and signing below you are indicating that you understand and authorize Albers Therapy Group Inc. to:

• charge your credit card when Occupational Therapy Services have been rendered. 

• charge your credit card for monthly recurring payments.

• charge your credit card in the case of last minute cancellations or "no-shows."

• charge your credit card to cover any fees and/or other purchases.

 

By signing you are also in agreemtent that your electronic signature has the same legal effect and can be enforced in the same way as a written signature.
